Actually my thought is why limit it to clutch failures only? If someone wants to set up an online database with data entry form and give me a URL I could have it display inline on this site, if I would take the time to learn PHP and MySQL better I would just do it myself.
Then we could keep all sorts of issues in the database and you could search it, run reports against it, etc. etc.
